Registration and electronic signature
You must provide complete and accurate information and upload only documents and images you are authorized to provide. A typed or drawn signature and the recorded terms version form your electronic acceptance. A parent or legal guardian must complete the consent for an applicant under 18.
Fees and Square
Fees displayed in the form are informational until validated by the authoritative server catalog. Square securely processes card information under its own terms. Universe For Cause receives a payment token, status, and transaction reference—not full card details. Submission does not guarantee acceptance, a title, advancement, or a refund.
Privacy, media, and retention
Applications may include identity documents, photographs, stories, ancestry information, and minors’ data. The privacy notice explains Azure processing, administrator access, Jotform migration, and administrator-controlled retention. Any competition or media release presented during registration applies in addition to these website terms.
